- Complete this form making sure to list ALL the samples being supplied and selecting the applicable tests.
- If you’ve received a quote, please make sure you reference this on the form as well.
- If you are a new client, please complete and attached a Customer Registration Form with your MAQR01 General Analysis Request Form – Microanalysis V12

How much sample do I need to send?
- If you’ve received a quote, you will find the amount required for each line item/test listed under required mass.
- If you haven’t received a quote, please click here for a list of tests and sample volume required (per sample). If you are unsure of the test that you want to have done, click here to send an enquiry outlining what you want tested, give us a call on (08) 9225 5810, or email admin@microanalysis.com.au.
Sending hazardous goods.
- If your samples are hazardous or you suspected them to be hazardous, please ensure that the packaging is correctly and clearly labelled and that they are accompanied by appropriate paperwork (preferably a current/up-to-date SDS) including a description of the hazard.
- All hazardous samples including suspected asbestos are to be double bagged (placed into a bag or container which is then placed into a second bag or container). This will assist in ensuring the safety of our staff and couriers.
Where do I send the samples to?
Making sure your samples are securely and correctly packaged, you can deliver, courier or use Australia Post to deliver to our sample receipt team at:

Please note that we cannot accept samples that are:
- Heavier than 15kg per item (unless previously arranged with our consultants).
- Incorrectly labelled or packaged.
- Leaking.
- Not accompanied by the required paperwork (completed MAQR02 General Analysis Request Form – Microanalysis Australia V12 and SDS if hazardous).
- If your delivery requires a truck, please note that we do not have a forklift available, so if you are sending a pallet, please use a tail lift truck.
